Support for Hosted Disk

Most everything (except advanced transport) is supported, except:

• The VixDiskLib_ConnectEx() extended connect function.
• SAN and HotAdd advanced transports.
• VixDiskLib_PrepareForAccess() and VixDiskLib_EndAccess() to delay Storage VMotion.
